  • Full medical examination including the eye test
  • Same day / next day availability
  • If you wear glasses/lenses please bring them along with the opticians prescription
  • You will need to bring a computer printout of your NHS Medical history summary/problem list and repeat medication list
  • If you have a chronic medical condition i.e diabetes, heart problems, Epilepsy etc you will need to bring along your full medical record
  • We will provide the PHV/204 medical application form
